- D 162 - Consumable and non-consumable medical material We request to supply material that will be a medical device meeting the specification in the appendix to this order. The purchased medical equipment must meet ALL PARAMETERS AND SPECIFIED REQUIREMENTS. Medical devices according to the law. 375/2022 Coll. submit a declaration of conformity in the EU according to the law. 90/2016 Coll. about conformity assessment of specified products when they are introduced to the market (CE mark, Declaration of Conformity) in both paper and electronic form. The content of the supplement must be a guide to use for each item (if published by the manufacturer) in the Czech language. The application for the offer also included a picture and a technical description of the offered material, which, in the event of a decision, would be submitted for comparative fulfillment of all specified parameters. Consumables must have an expiration date of at least 18 months from the date of delivery to the buyer. If the manufacturer states the shelf life of times, then the material must be delivered, its shelf life will be at least 2/3 in favor of the buyer from the date before the buyer. In the event that due to determinable reasons it is not possible to determine the shelf life, it is necessary to consult with the buyer before calculating the delivery.
