Description:
The subject of this public contract is the continuous supply of catheter-implanted wireless pacemakers, including accessories intended for the treatment of heart rhythm disorders (hereafter referred to as "SZM") for KKVC for adults - Department of Cardiology 2. LF UK and FN Motol, within the scope of the AppendixNo. 1 Purchase contracts List of goods with unit prices. The subject of public contract performance is defined in Appendix No. 2 of the procurement documentation - Technical specification of the subject of performance and in Appendix No. 3 of the procurement documentation - Information and price table.
The contracting authority will use catheterization implanted wirelesscardiac pacemakers for effective and emergency applications, therefore requests from the supplier a consignment warehouse at its workplace, through which the supplier will deliver the goods that will be the subject of the concluded Contract. The binding terms and conditions of the consignment are listed in Appendix No. 2 of the Purchase Agreement - Scontract for the establishment and management of a consignment warehouse.
Part of the subject of performance is also the free loan of programming equipment for the offered catheterization-implanted wireless pacemaker. The binding terms and conditions of the loan are listed in Appendix No. 3 of the Purchase Agreement - Loan Agreement.
