• Description:
• This is a small-scale public contract (hereafter referred to as VZMR) for the purchase of consumable diagnostic material for laboratory instruments of IDEXX Laboratories s.r.o. for blood testing for the Hlučin Military Veterinary Institute for the purpose of securing the professional activities of the VVetU blood testing of service phorses and sport horses of ASC Dukla Prague as part of disease diagnostics and preventive examinations. IDEXX consumables cannot be replaced with alternative material from another company due to incompatibility with IDEXX Laboratories s.r.o. devices. The proposed supplier is an exclusive supplierof the required material in the Czech Republic, which is evidenced by the "Declaration of Honor" in the annex to the market survey. The delivery of goods with a shorter expiration date must be discussed with the customer (contact person) in advance.
  
  The purchase of goods will be carried out according to demandof the client's payments on the basis of a concluded one-time purchase contract (KS) with the winning bidder VZMR.
  
  Conclusion of a purchase contract with fulfillment in two stages:
  - the first stage with the delivery date of the goods by 30.4.2023 with a minimum expiration of 9 months from the delivery of the goods
  - the second stage with the delivery date of the goods from10/9 to 10/12/2023 with a minimum expiration of 9 months from the delivery of the goods
  
  The list of required goods for valuation is given in Appendix No. 2-Price offer-valuation. For items that will not be charged ("zero" price), a donation contract will subsequently be issued.
  
  Requirement:
  Participant in the form of attachments in the VZMR (supplier's attachments) will submit professional qualification prerequisites according to § 77 Z ZVZ 134/2016, paragraph 1, paragraph 2 p. and). extract from the commercial register and trade register. For §77, if the required letters from the given paragraphs are not listed - see mentioned above (Section 77 paragraph 1, paragraph 2 letter a), mayto exclude the participant from VZMR.
  Further filling in the price offer in the table - see. appendix no. 2 ATTENTION start according to the instructions under the table in appendix no. 2. As appendix no. 3, insert the discussed and possibly modified draft of the purchase contract, which will be signed by a person authorized to act on behalf of the participant's organization. Parti attachment no. 3 is an attachment in Excel format of 2 sheets (I. stage and II. stage of delivery of goods) no. 1 to the purchase contract No. SpMO 25722/2023-684809/.., which must also be filled in. is the exclusive supplier of the required material for the Czech Republic. We request the submission of the Affidavit of Power of Attorney for representationand the distribution of the given company for the Czech Republic for the year 2023 in the attachment section.
  
  Submit the required attachments in PDF format. The required documents proving the fulfillment of the basic qualification prerequisites and extracts from the commercial and trade register must not be older than 3 months from the date of the announcement of the VZMR.
  
  Further onwe require a statement from the supplier - see Appendix No. 4 - Supplier's declaration of the socially responsible performance of a public contract in accordance with the Resolution of the Government of the Czech Republic No. 531 of 24 July 2017, on the Rules for the Application of a Responsible Approach in the Awarding of Public Contracts and Purchases of State Administration and Local Government.
